Medication quality inconsistent across refills — two different compounds, different response
Four months in. The first three refills felt consistent in effect and tolerability. My fourth refill was noticeably different — injection site stinging that I hadn't had before and a two-week period where the appetite suppression felt weaker. When I messaged Peter MD about this, the reply acknowledged compounding pharmacies can occasionally use different formulations and suggested I continue the current batch and let them know if issues persisted. The issue did persist, and the reply to my follow-up was a suggestion to try a different injection technique. I've lost 9 lbs total but stalled in the last two months. I can't prove the compounding quality was the issue, but the correlation was hard to ignore. For consistency across refills, this is a real risk.

No labs is a real gap at higher doses — but the service itself is excellent
Three months on tirzepatide, 2.5mg to 5mg titration, down 18 lbs. The service is efficient, the pricing is transparent, and my questions have been answered fast. The only concern I have is no labs included or coordinated. At higher tirzepatide doses I'd like to see periodic metabolic panels. My PCP agreed to run labs and the results were fine, but the coordination was on me — not everyone has a cooperative PCP and not everyone will think to ask. Peter MD should at minimum recommend timing for lab monitoring. For patients without complex histories, this probably doesn't matter. For anyone with diabetes, elevated A1c, or cardiovascular factors, go somewhere with lab support.

I was skeptical of async-only telehealth but Peter MD has proven the model works for me. I ask maybe two questions a month through the portal and both get answered the same day. The titration guidance was clear from day one — I got a written protocol explaining when to dose-escalate and what side effects are dose-dependent versus concerning. I followed the protocol, escalated from 0.25mg to 1mg over five months, and I'm down 28 lbs. No labs included — I get my own annually through my PCP and haven't needed anything different. For a standard GLP-1 weight loss goal, Peter MD does the job at a competitive price.

Competitive pricing and fast approval — limited to semaglutide and tirzepatide
For GLP-1 weight loss specifically, Peter MD is well-priced and well-run. Fast intake, quick approval, competitive monthly cost, provider messaging that actually works. The limitation is catalog breadth — if you want to add peptides beyond the GLP-1 family, Peter MD doesn't offer them. I'm using semaglutide 1mg and it's effective — down 23 lbs in four months. But I've been researching CJC-1295 for body composition and will have to go elsewhere for that. For patients whose goal is purely weight loss with GLP-1s, this service does everything right. For patients who want a broader peptide program, plan on needing a second provider.

Medication works but shipping delay caused a two-week gap
My first three months were smooth — onboarding was fast, medication was good, lost 12 lbs. Month four, my refill order took 11 days. No tracking update between day 5 and day 10. I ran out of semaglutide and went two weeks without it. I messaged support twice during the gap and got an automated response the first time and a real reply on day 8 saying the pharmacy had a fulfillment delay. I had to restart at a lower dose because of the extended break, which set my progress back. The medication and pricing are solid. The shipping reliability at month four and the support responsiveness during the gap are why this isn't a higher rating.

5-7 day shipping is the one real limitation — otherwise very good
Three months in, 15 lbs down, pricing is excellent and the provider communication has been solid. The only meaningful complaint is shipping time. 5-7 days sounds reasonable until you're on a monthly refill cycle and a delayed shipment means a gap in your medication schedule. My second refill arrived on day 8 and I missed one injection. The provider's advice was to just skip that week and continue — not ideal for someone trying to maintain consistent blood levels. If they got shipping to 3-4 days, this would be a five-star service. The clinical and pricing side is excellent. The logistics is the one area where they lose to competitors.

The reply came within 12 hours and said to rotate sites more aggressively and consider a different injection technique. That advice is fine as far as it goes. But I also wanted to know whether this is a formulation issue, whether I should consider a different compounding pharmacy, whether the reaction was worth monitoring. The response was generic. There's no way to escalate to a longer clinical conversation without going elsewhere. Not a bad service — just a limited one.
